The Many Complications Of Ovarian Cysts

Tubal pregnancies - this is called an ectopic. Once in a while the pregnancy develops in the tube before it makes it down and these conditions are life threatening. Most of them are not, but in the process the tube can fill up with blood and before it ruptures. And this can also appear on ultrasound as a cyst and it can feel like a cyst. Most of the time a pregnancy test will be positive, and when you combine that with the enlarged tube, well you’ve got your diagnosis and saved an emergency.

We’ve talked about endometriosis, that which can be a cyst. They’re rare types of colon tumors, most of these are solid bowel abscesses, can be sometimes diverticulosis, diverticulitis. You can have abscesses that can feel - most of these, well they can feel, or present like a cyst, and they’re going to be painful. Most ovary cysts in and of themselves are not going to be causing a problem. And there are a couple of other conditions that can be mistaken for a pelvic tumor. They’re not cysts but I’m going to mention them; uterus fibroids or uterine myomas, and pelvic kidney’s. The kidney actually migrates during development. A lot of strange stuff happens to us. We’re lucky that so many of us are normal. The pelvic kidney sometimes remains in the pelvis and can feel like a tumor until the proper tests are done to identify it as a kidney. So, during the work up process all these things are considered. But, the important thing here, is that cysts themselves, in and of themselves are not going to be a problem or life threatening problem.

In his white paper, New York OB/GYN Christopher Freville discusses the complications of cysts. What are the complications of the cyst? Well, first and foremost if any cyst, any of these types of cysts we talked about; the benign ones can cause a problem if they get so large that they start twisting, called torsion. And, when an ovary generally gets bigger than 10 centimeters that can be a problem. I’ve seen it less than that, but when the ovary starts twisting itself just imagine a tube with a big ball on the end and by movement or whatever this tube gets twisted and twisted. You ultimately end up shutting down the blood supply and causing that particular ovary tube, called the addnexit to loose it’s blood supply, become necrotic, which means tissue dies, and it has to be removed. Well, prior to that process it’s extremely painful. Most of the time those torsed ovaries cannot be saved. Which is not a problem because just like kidneys, just like testis, women with one ovary can have just as many babies as a woman with two ovaries and two tubes. So it doesn’t decrease the fertility by half which is a common misconception.

So we’ve got torsion as a complication. Obviously the pain is a definite complication and if the pain persists, well it’s time to have something done. The complications of endometriomas of course, and polycysticomas; the most common ones are infertility. The endometriomas, if they persist, and there’s continuous bleeding in the abdomen and the ovary that can cause adhesions. Any bleeding cyst can be a problem. Any cyst can have a blood supply. Most of these, the bigger blood supplies, the bigger blood vessels on cysts are usually the corpus luteum cyst. A pregnancy with a corpus luteum, the second type of functional cyst goes on to develop as the baby is growing. The blood supply is very, very rich and they break, they burst, they rupture a vessel can pump blood into the abdomen and can be a life threatening condition. The same thing can be true with, certainly with tube pregnancies, that’s why those two can be confused. So, you can have bleeding with a cyst but most of the time those are rare circumstances. Torsion, bleeding, when it ruptures most of the time there’s going to be pain.

Follicular Cysts And Corpus Luteum Cysts

A cyst can rupture be it a, most likely it’s a follicular cyst or a corpus luteum cyst. These are the so-called functional cysts, or the functional cyst with pregnancy. When they rupture generally nothing happens. In fact, there’s usually a pressure relief.

In a severe case woman can go into shock meaning their blood pressure falls out, it’s an emergency, life threatening situation that if undiagnosed can be fatal. Fortunately, with emergency rooms being equipped the way they are, doctors being as smart as we are - we rarely miss this. But once in a great while this is a complication. And when the cyst ruptures it doesn’t, you can usually preserve the ovary. Usually it can be sutured over, quarterized. The ovary stays in tact and even in a rare case if it had to be removed, again that does not affect fertility.

You will need to take a sample of your semen to the clinic for testing. This will look at the numbers of sperm, how they move and whether they have a normal structure.

Treatment

The treatment that is best for you will depend on the cause of subfertility.

Helping you to ovulate

If all or part of the subfertility is to do with not ovulating (eg if you have polycystic ovary syndrome), treatments can stimulate your inactive ovaries to produce eggs. This is called ovulation induction. At its simplest, this involves taking a medicine called clomiphene (eg Clomid) or tamoxifen for six days each month. Your doctor may also prescribe a medicine called metformin if you have polycystic ovary syndrome and clomiphene wasn’t effective on its own.

Controlling ovulation and collecting eggs

A more complex form of ovulation induction is used if you are preparing for the fertility treatments called in-vitro fertilisation (IVF) and intracytoplasmic sperm injection (ICSI). The aim is to control the timing of your monthly cycle accurately so that your eggs can be removed and fertilised on a specific day. A normal monthly cycle will produce only one egg, but with this method, which uses three hormones given at different times, you produce several ripe eggs at once. This is called superovulation and it increases your chances of a pregnancy.

Usually, the eggs are collected while you have an ultrasound scan. A thin needle is passed through your vagina and into your ovary. This is done as an outpatient procedure, so you won’t need to stay overnight in hospital and you usually won’t need a general anaesthetic.

Assisted conception

There are many different technologies that can help you to conceive. All of these aim to bring the sperm and an egg, or eggs, close together. The three main methods are as follows.

Intra-uterine insemination (IUI for short)

Sperm that have been washed and specially prepared are put inside your womb just before ovulation is due. This is usually the first method offered to couples who have unexplained infertility. It’s also useful if there are ejaculation problems or mild problems with the quality of the sperm. IUI can be combined with ovulation induction (see Helping you to ovulate).

In vitro fertilisation (IVF)

IVF can be carried out with donor sperm or donor eggs. The sperm and embryos can also be frozen for future use, although there are strict legal rules, supervised by the HFEA, to regulate these procedures. You will need to discuss these methods with your specialist.

In the UK, the Human Fertilisation and Embryology Authority (HFEA) licenses clinics offering advanced fertility treatments such as IVF.

Eggs and sperm are mixed in a test tube in a laboratory - this is where the phrase “test-tube baby” comes from. Successful fertilisation can be seen with a microscope after about 12 to 20 hours. One or two embryos are transferred into your womb using a soft plastic tube passed through your vagina. IVF is useful if your fallopian tubes are damaged or have been removed. It can also work well if your subfertility is caused by endometriosis (for women) and for mild sperm problems (for men).

Intracytoplasmic sperm injection (ICSI)

A single sperm is injected into an egg in the laboratory and the resulting embryo is transferred to the womb, as with IVF. The cytoplasm is just the scientific name for the insides of the egg cell. This type of fertility treatment is used when there are more serious problems with the sperm or when problems between the egg and sperm are preventing fertilisation.

Complications

Complications are when problems occur during or after the treatment.

Ovulation induction increases the chances of having a multiple pregnancy (such as twins). This raises the risk of problems for the mother and baby.

Ovulation induction can also produce a rare condition called ovarian hyperstimulation syndrome, in which the ovary is overstimulated and produces too much of the hormone oestrogen. As a result, the woman is at an increased risk of getting blood clots and chest problems, and may need to go into hospital for monitoring and treatment.

Pregnancies that result from IVF can still be ectopic (the embryo starts to develop outside the womb, usually in a fallopian tube). Also, any technique that involves putting instruments into your body can cause infection or damage to internal organs.

Success rates

The success of assisted conception is very variable and depends on the reason for subfertility and, for women, your age. The success rate goes down sharply over the age of 40. If you are having IVF or ICSI, your clinic should tell you its live birth rate - the so-called “take home baby” rate. Each year the figures for the UK are published by the HFEA (see Further information).

An Interesting Cause of Yeast Infection

If you were to ask 100 people what the cause of yeast infection is, most would be able to give you the more common reasons. However, the most common answer may not be the most common cause for yeast infection. For example, in women, the primary cause of yeast infection has to do with hormones. In this case, when estrogen levels in the body are higher than normal, Candida yeast multiplies to unhealthy levels. These estrogen levels are usually increased during a woman’s menstrual cycle, from pregnancy, taking hormone replacement therapy, or even oral contraceptives.

Causes of recurring yeast infections also have to do with lifestyle and climate, something of which many people are unaware. For instance, individuals living in tropical or subtropical climates are generally more prone to yeast infections. We suppose a price has to be paid for living in a warm paradise - but does sacrifice have to be what causes a yeast infection Even clothing can contribute to the start of a yeast infection. The vaginal and penile areas need proper ventilation. Without it, moisture and warmth provide the ideal place for Candida to grow and even flourish. The good news is that you can fix this cause of yeast infection simply by choosing cotton undergarments, looser fitting pants, and apparel that offers better air circulation.

It is also interesting to learn that some diseases such as diabetes can be associated with the cause of recurrent yeast infections. While women are at risk, studies show that the risk for men is actually much higher. In this case, sugar in the bloodstream rises, which yeast loves. Therefore, if you have diabetes or experience recurring yeast infections, talk to your doctor to determine the actual cause and remedy.

Remember, no matter the reason or cause of chronic yeast infections, most cases can be treated and controlled quite easily and inexpensively. On the other hand, allowing the problem to continue without treatment could lead to severe symptoms so take action immediately.
